The aim of this study was to assess the association between multimorbidity and the presence of functional limitation in basic (BADL) and instrumental activities of daily living (IADL) among Brazilian older adults and to verify whether this association is moderated by socioeconomic conditions. Cross-sectional study with data from the Brazilian National Health Survey (PNS) (2019) for the Brazilian population aged 60 years and over. The dependent variables were functional limitation, based on self-reported difficulty in performing one or more activities of daily living, including six BADL (feeding, bathing, using the toilet, dressing, crossing a room on the same floor and getting out of bed) and four IADL (shopping, managing money, taking medication and using transportation). The independent variables were multimorbidity (presence of two or more self-reported chronic diseases) and socioeconomic measures (per capita household income, asset score, and education level). The association between multimorbidity and outcomes was assessed using adjusted logistic regression models. The moderating effect of socioeconomic conditions on the association between multimorbidity and functional limitations was assessed by including an interaction term. The final sample consisted of 22,725 individuals. The prevalence of functional limitation was 8.5% (95%CI: 7.9-9.2) and 18.6% (95%CI: 17.8-19.5) in BADL and IADL, respectively. Multimorbidity was associated with BADL [OR: 2.30 (95%CI: 1.93-2.74)] and IADL [OR: 2.26 (95%CI: 1.98-2.57)]. The odds of functional limitation were higher among individuals with lower levels of education and income, but there was no interaction between multimorbidity and socioeconomic position measures. Multimorbidity was associated with functional limitation (BADL and IADL) and socioeconomic conditions, and this association was constant across socioeconomic position levels.

ABSTRACT Objective: To evaluate the oral health and related factors among people with visual impairment in a municipality in southeastern Brazil. Methods: A cross-sectional and descriptive household study was carried out on a representative sample of people with visual impairments in a medium-sized city in Brazil. A clinical-dental examination was performed (evaluating tooth loss, caries, periodontal health, dental plaque use and need of prosthesis) and a semistructured questionnaire was applied, investigating the socioeconomic conditions (sex, age, marital status, skin color and income) and the use of dental services (reason, time since, and type of service used [private/public] in last dental visit). Descriptive and bivariate analysis were performed to asses the factors related to oral health. Results: The mean age of participants was 65 years (±15.05), with the majority women (70.9%). 53.1% of the interviewees were edentulous, but 58.2% had a good perception of oral health. The blind ones use less public services (p=0.038) and with a lower frequency (p=0.014) than subjects with low vision. Those with less schooling had higher tooth loss and lower periodontal problems (p <0.05). There was a reduction of dental plaque six months after the educational action (30.7%; p=0.01), especially among the low vision participants (p=0.01). Conclusion: Severe tooth loss and unsatisfactory hygiene contrast with self-perception of oral health. Visual acuity and schooling were related to access and oral health. Actions for promotion and equity are necessary to overcome the inequalities encountered.


RESUMO Objetivo: Avaliar a saúde bucal e fatores relacionados entre deficientes visuais em município do sudeste brasileiro. Métodos: Foi realizado um estudo domiciliar, transversal e descritivo em uma amostra representativa de pessoas com deficiência visual em uma cidade de médio porte do Brasil. Realizou-se o exame clínico bucal (avaliando perda dentária, cárie, saúde periodontal, placa dentária, uso e necessidade de prótese) e aplicou-se um questionário semiestruturado, investigando as condições socioeconômicas (sexo, idade, estado civil, cor da pele e renda) e o uso de serviços odontológicos (motivo, tempo decorrido e tipo de serviço utilizado [privado/público] na última consulta odontológica). Foram realizadas análises descritivas e bivariadas para avaliar os fatores associados à saúde bucal. Resultados: A idade média dos participantes foi de 65 anos (±15,05), com maioria de mulheres (70,9%). 53,1% dos entrevistados eram edêntulos, porém, 58,2% apresentaram percepção de saúde bucal boa. Os cegos usam menos os serviços públicos (p=0,038) e em menor frequência (p=0,014) que os sujeitos com baixa visão. Os menos escolarizados apresentam maiores perdas dentárias e menores problemas periodontais (p<0,05). Houve redução de placa dentária seis meses após ação educativa (30,7%; p=0,01), sobretudo entre os sujeitos com baixa visão (p=0,01). Conclusão: Elevada perda dentária e higienização insatisfatória contrastaram com autopercepção dos agravos bucais. Acuidade visual e escolaridade relacionaram-se ao acesso e saúde bucal. Ações de promoção e equidade são necessários para superar desigualdades encontradas.

Resumo Introdução Pacientes com insuficiência renal crônica podem apresentar prejuízos em sua saúde bucal em decorrência da própria doença, do tratamento e das alterações de estilo de vida associadas. Objetivo Avaliar os fatores associados à autoavaliação de saúde bucal ruim entre adultos com insuficiência renal crônica submetidos à hemodiálise. Método Estudo transversal com 243 adultos submetidos à hemodiálise em um hospital do sul de Minas Gerais nos anos de 2013 e 2014. O desfecho foi avaliado pelo autorrelato da condição bucal dicotomizada em boa (ótima/boa) e ruim (regular/ruim/péssima). As variáveis independentes incluíram condições sociodemográficas, saúde geral, saúde bucal e uso de serviços odontológicos, a partir de informações coletadas por meio de questionário. A associação entre o desfecho e as variáveis independentes foi testada por meio de modelos logísticos múltiplos com inclusão hierarquizada de variáveis. Resultados A prevalência de autoavaliação de saúde bucal ruim foi de 35,4%. Os mais jovens (p = 0,015), os que se submetem à hemodiálise há menos tempo (p = 0,016), têm halitose (p <0,001), necessitam de tratamento odontológico (p <0,001) e tiveram a última consulta odontológica por motivo diferente de dor (p = 0,027) expressaram maiores chances de autoavaliação de saúde bucal ruim, independentemente de condições sociodemográficas e de saúde. Conclusão Condições sociodemográficas, tempo em hemodiálise, agravos à saúde bucal e uso de serviços odontológicos influenciaram a autoavaliação da saúde bucal dos adultos submetidos à hemodiálise.


Abstract Background Patients with chronic renal failure may have their oral health impaired as a result of the disease itself, its treatment, and its associated lifestyle alterations. Objective To assess the factors associated with poor self-rated oral health among adults with chronic renal failure treated by hemodialysis. Method This is a cross-sectional study with 243 adults undergoing hemodialysis in a hospital in Minas Gerais, Brazil in 2013-2014. The outcome was assessed by the self-report of oral health categorized into good (excellent/good) and bad (fair/bad / very bad). The independent variables included sociodemographic conditions, general health, oral health, and the use of dental services were collected through a structured questionnaire. The association between the outcome and the independent variables was tested using multiple logistic models with hierarchical inclusion of variables. Results The prevalence of poor self-rated oral health was 35.4%. The youngest (p = 0.015), those who have undergone hemodialysis in the shortest time (p = 0.016), have halitosis (p <0.001), need dental treatment (p <0.001), and had their last dental appointment not for pain (p = 0.027) expressed higher odds of poor self-rated oral health, independently of sociodemographic and health conditions. Conclusion Sociodemographic conditions, time on hemodialysis, oral impairments, and use of dental services affected the oral health self-assessment among adults undergoing hemodialysis.

Introdução: a busca por atendimento odontológico é um parâmetro importante para averiguar como os indivíduos compreendem a saúde bucal. Objetivo: avaliar a utilização de serviços odontológicos e fatores associados em pacientes oncológicos em Minas Gerais, Brasil. Material e método: estudo transversal, realizado com uma amostra de 441 pacientes adultos frequentadores de uma associação voluntária de apoio a pacientes oncológicos no Estado de Minas Gerais, Brasil. O desfecho foi avaliado por meio do autorrelato de consulta odontológica no último ano. As variáveis independentes incluíram condições socioeconômicas, saúde geral e relacionada ao câncer, saúde bucal e cuidados odontológicos. As associações entre o desfecho e as variáveis independentes foram testadas por meio de regressão logística bivariada e múltipla, sendo os resultados expressos por meio de odds ratio (OR) e intervalos de confiança de 95% (IC95%). Resultado: a prevalência de consulta odontológica no último ano foi de 33,3%. Os indivíduos que possuem entre quatro e sete (OR: 1,80; IC95%: 1,02-3,18) e aqueles com oito ou mais anos de estudo (OR: 3,20; IC95%: 1,07-9,60) apresentaram maiores, enquanto os edêntulos (OR: 0,44; IC95%: 0,21-0,92) apresentaram menores chances de consulta odontológica no último ano, independentemente de condições socioeconômicas, saúde geral e saúde bucal. Conclusão: foi baixa a procura por consulta odontológica no último ano entre os pacientes oncológicos. Além disso, houve desigualdades relacionadas à escolaridade e ao número de dentes remanescentes na utilização dos serviços. A identificação e priorização dos grupos com acesso desfavorável aos serviços são fundamentais no planejamento da prestação de cuidados odontológicos a essa população.


Introduction: the search for dental care is an important parameter to determine how individuals understand the oral health. Objective: to evaluate the use of dental services and associated factors in cancer patients in Minas Gerais, Brazil. Material and method: a cross-sectional study carried out with a sample of 441 adult patients attending a voluntary association to support cancer patients in the State of Minas Gerais, Brazil. The outcome was assessed through self-report of dental appointments in the last year. Independent variables included socioeconomic status, general and cancer-related health, oral health, and dental care. The associations between the outcome and the independent variables were tested using bivariate and multiple logistic regression, and the results were expressed as odds ratios (OR) and 95% confidence intervals (95%CI). Result: the prevalence of dental appointments in the last year was 33.3%. Individuals who have between four and seven (OR: 1.80; 95%CI: 1.02-3.18) and those who have eight or more years of schooling (OR: 3.20; 95%CI: 1.07-9 .60) had greater, whereas, the edentulous (OR: 0.44; 95%CI: 0.21-0.92) had less odds to have had a dental appointment in the last year, independently of socioeconomic status, general health and oral health conditions. Conclusion: recent dental care was uncommon among cancer patients. Furthermore, there were inequalities related to schooling and the number of remaining teeth in the use of services. The identification and prioritization of groups with unfavorable access to services are fundamental in planning the provision of dental care to this population.

AIM: To assess edentulism-free life expectancy (EFLE) and the related inequalities by sex and schooling among older Brazilian adults from 2006 to 2016. BACKGROUND: Tooth loss is related to shortened longevity and unhealthy life expectancy in old age. MATERIALS AND METHODS: The outcome of the study was EFLE, assessed by age, sex and schooling. EFLE was estimated using the Sullivan method, considering the years and proportion of remaining life and the prevalence of edentulism-assessed in the Health, Well-being, and Aging cohort study, as well as the official mortality data for adults aged 60 years or older living in São Paulo, Brazil. RESULTS: EFLE increased from 10.9 (95% CI: 10.4-11.5) to 13.8 (95% CI: 13.2-14.5) years, considering data from 2006 to 2016, among 60-year-old individuals. In relative terms, these individuals expected to live 50.7% (95% CI: 48.1-53.2) of their remaining life free of edentulism in 2006, while this expectation was 62.8% (95% CI: 60.0-65.6) in 2016. Within both years, women and the less educated had lower EFLE than men and the higher educated. CONCLUSION: EFLE increased from 2006 to 2016. However, inequalities concerning sex and education remained significant, thereby highlighting the need to continuously address inequalities in tooth loss throughout life to contribute to a healthy ageing.

Oral impairments can affect overall health and life expectancy in older adults. Our study evaluates the life expectancy with negative physical oral health impact on quality of life (POHIQoL) among older adults. Life expectancy with negative POHIQoL was estimated by the Sullivan method, using the prevalence of POHIQoL - obtained in the Health, Well-being and Ageing (SABE Study); and official mortality data for adults aged 60 years or older living in São Paulo, Brazil. Between 2000 and 2010, negative POHIQoL increased from 23.4% (95%CI: 20.2-26.9) to 30.4% (95%CI: 27.0-34.3) among older adults; total life expectancy increased from 22 and 17.5 to 23.7 and 19.4 years among 60-year-old women and men, respectively; and the proportion of remaining years to be lived with negative POHIQoL increased from 25.1% to 32.1% for the same age group. In both years, individuals aged 60 years with lower education level were expected to live more years with negative POHIQoL when compared with the most schooled ones (2000: 15.9 [95%CI: 15.0-16.8] vs. 14.3 [95%CI: 13.7-14.8]; 2010: 16.3 [95%CI: 15.1-17.4] vs. 14.1 [95%CI: 13.2-15.1]). Similarly, women were expected to live more years with negative POHIQoL than men. Within ten years, life expectancy with negative POHIQoL increased, as well as the existence of inequalities in sex and education level among Brazilian older adults. Expansion in coverage and focus on equity in dental care are still necessary to overcome persistent dental-related problems and inequalities and, therefore, contribute to healthy ageing.

We assessed the association between edentulism and all-cause mortality among community-dwelling older adults from São Paulo, Brazil, from 2006 to 2017. This prospective cohort study used data from the Health, Well-being and Aging Study (SABE, Portuguese acronym). Edentulism was evaluated by means of clinical oral examination and all-cause mortality data were obtained from state official records. Covariates included socioeconomic factors (age, sex, and schooling); health behavior (smoking, alcohol intake, and physical activity); dental care (prostheses use); general health (multimorbidity); and nutritional status (underweight). Kaplan-Meier survival curves were stratified by edentulism and compared using the log-rank test. Cox proportional hazards model was applied to calculate hazard ratios (HRs) for the association between edentulism and mortality after adjusting for covariates. The study sample included 1,687 participants (age, 60-102 years; edentulous: 47.2%). In the 11 years of follow-up, we analyzed 10,494 person-years and 566 deaths. In bivariate analysis, edentulous older adults were found to be at a higher risk of dying from all causes than the dentate participants (HR: 1.81; 95%CI: 1.53-2.15). After sequential adjustment for socioeconomic factors, health behavior, dental care, general health, and nutritional status, this association was attenuated, but remained significant (HR: 1.34; 95%CI: 1.10-1.63). In conclusion, edentulism is a significant predictor of all-cause mortality among older adults.

Resumo: As doenças bucais podem afetar a saúde geral e a expectativa de vida dos idosos. O estudo avalia a expectativa de vida com impacto negativo da saúde bucal física sobre a qualidade de vida (POHIQoL, por sua sigla em inglês) em idosos. A expectativa de vida com POHIQoL negativa foi estimada pelo método de Sullivan, usando a prevalência de POHIQoL, obtida no estudo Saúde, Bem-Estar e Envelhecimento (Estudo SABE), e dados de mortalidade oficiais para adultos com idade de 60 anos ou mais e residentes em São Paulo, Brasil. Entre 2000 e 2010, a POHIQoL negativa aumentou de 23,4% (IC95%: 20,2-26,9) para 30,4% (IC95%: 27,0-34,3) entre idosos. Nos indivíduos com 60 anos, a expectativa de vida total era maior em mulheres do que em homens em 2000 (22 e 17,5 anos, respectivamente) e em 2010 (23,7 e 19,4 anos respectivamente). A proporção de indivíduos com anos de vida remanescentes com POHIQoL negativa aumentou de 25,1% para 32,1% no mesmo período. Os indivíduos com 60 anos com menor escolaridade viveriam menos anos com POHIQoL negativa, comparados aos com escolaridade maior (2000: 15,9 [IC95%: 15,0-16,8] vs. 14,3 [IC95%: 13,7-14,8]; 2010: 16,3 [IC95%: 15,1-17,4] vs. 14,1 [IC95%: 13,2-15,1]). Além disso, as mulheres poderiam esperar viver mais anos com POHIQoL negativa do que os homens. Entre 2000 e 2010, houve um aumento na expectativa de vida com POHIQoL negativa, além da existência de desigualdades por gênero e escolaridade, entre os idosos brasileiros. A ampliação da cobertura e o foco na assistência odontológica ainda são necessários para superar os problemas persistentes de saúde bucal e as desigualdades associadas, e assim, contribuir para o envelhecimento saudável.


Resumen: Los trastornos orales pueden afectar sobretodo la salud y esperanza de vida de adultos mayores. El estudio evalúa la esperanza de vida, asociada al impacto de una salud bucodental negativa que afecta a la calidad de vida (POHIQoL, por su sigla en inglés) de adultos mayores. La esperanza de vida con una POHIQoL negativa se estimó mediante el método Sullivan, usando: prevalencia de POHIQoL -obtenida en Salud, Bienestar y Envejecimiento (Estudio SABE); y los datos oficiales de mortalidad para adultos con edades comprendidas entre 60 años o más, que viven en São Paulo, Brasil. De 2000 a 2010, la POHIQoL negativa se incrementó de 23,4% (IC95%: 20,2-26,9) a 30,4% (IC95%: 27,0-34,3) entre adultos mayores; la esperanza de vida total se incrementó de 22 y 17,5 a 23,7 y 19,4 años entre mujeres y hombres con 60 años, respectivamente; y la proporción de años restantes de vida con una POHIQoL negativa, se incrementó de 25,1% a 32,1% para el mismo grupo. Las personas con una edad de 60 años y bajos niveles de escolaridad vivirían más años con una POHIQoL negativa, si los comparamos con los más escolarizados (2000: 15,9 [IC95%: 15,0-16,8] vs. 14,3 [IC95%: 13,7-14,8]; 2010: 16,3 [IC95%: 15,1-17,4] vs. 14.1 [IC95%: 13,2-15,1]). Igualmente, las mujeres vivirían más años con una POHIQoL negativa que los hombres. Hubo un incremento en la esperanza de vida con POHIQoL negativa desde el 2000 al 2010, junto con la existencia de inequidades por sexo y escolaridad entre adultos brasileños mayores. La expansión de la cobertura dental, enfocándonos en la equidad de su cuidado dental, es necesaria para superar los problemas dentales persistentes y las inequidades descritas, con el fin de contribuir, de esta forma, a un envejecimiento saludable de la población.

Aim: To assess oral health-related quality of life (OHRQoL) and associated factors among the 12-year-old population of the state of Minas Gerais, Brazil. Methods: Cross-sectional data from the SB-Minas Gerais 2012 study were used. The presence of poor OHRQoL was assessed using the Oral Impact on Daily Performance (OIDP) and its dimensions (physical, psychological and social domains). Independent variables included sociodemographic factors and variables related to the use of dental care and oral health conditions. The association between the outcomes and the independent variables were tested using logistic regression and the results reported as odds ratio with 95% confidence interval. Results: Prevalence of poor OHRQoL was 31.4%; the psychological domain was the most affected (22.6%). Pain and dissatisfaction with oral health were associated with poor OHRQoL on overall OIDP and all its domains. Non-whites had greater poor OHRQoL than whites on overall OIDP and physical domain. Conclusion: Self-perceived oral health and social inequalities were associated with poor OHRQoL

The aim of this study was to evaluate the relationship between the perception of dental care services and oral health conditions in a hypertensive and diabetic population subscribed to the Family Health Strategy in the city of Alfenas, Minas Gerais, Brazil. This was a domiciliary, descriptive-analytical study with random and stratified sample, consisting of 186 individuals. The following indexes were applied: Decayed, Missing and Filled Teeth (DMFT); Filled and Sound Teeth (FS-T); Dental Care Index (DCI); Evaluation of the use and need of dentures; Oral health for primary care assessment questionnaire (OHPCA). A predominance of women (67.74%), hypertensive non-diabetic people (58.60%) and elderly people (52.69%), with an average age of 64.26 (± 12.22) years, was observed. We noticed a total DMFT of 27.00 (± 6.24); FS-T = 8.94 (± 10.28); DCI = 19.42 (± 26.80); 39.78% of partial dentures usage, 56.45% of total prosthesis usage with necessity of 36.02% of partial dentures and 28.49% of total prosthesis. In the OHPCA questionnaire, positive assessments of services were prevalent. Oral Health proved to be best among those individuals who reported good relationship with their dentists and worse among those individuals who considered as good the equipment used in the services. The critical aspects were: access, speed and professional-patient communication.

Aplicaram-se os índices: Dentes Cariados, Perdidos e Obturados (CPOD); Índice de Dentes Funcionais (FS-T); Índice de Cuidados Odontológicos (ICO); Avaliação do uso e necessidade de próteses; Questionário de Avaliação da Saúde Bucal na Atenção Primária (ASBAP-usuário). Observou-se predomínio de mulheres (67,74%), hipertensos não diabéticos (58,60%) e idosos (52,69%) idade média de 64,26 (± 12,22) anos. Registrou-se CPOD = 27,00 (± 6,24); FS-T=8,94 (± 10,28); ICO=19,42 (± 26,80); 39,78% de uso de próteses parciais, 56,45% de próteses totais e necessidades de 36,02% de próteses parciais e 28,49% de próteses totais. No ASBAP-usuário, prevaleceram avaliações positivas dos serviços. A Saúde Bucal mostrou-se melhor entre os sujeitos que relataram bom vínculo com o dentista e pior entre os que consideraram bons os equipamentos utilizados nos Serviços, sendo ainda pontos críticos: acesso, rapidez e comunicação profissional-paciente.

This research aimed to assess the oral health related to quality of life among hypertensive and diabetic patients in the city of Alfenas, Brazil. This was a domiciliary-based, descriptive-analytical, cross-sectional research with a random, systematic sample stratified by the Family Health Team, consisting of 218 individuals. The following indexes were applied: DMFT, T-Health, FS-T, SiC index, use and need of prosthesis and OHIP-14. Most of the patients (56.42%) had only high blood pressure, were females (67.43%), with an average age of 64.83 (±11.99) years old, varying between 35 and 93 years old. No significant differences on the variables between hypertensives, diabetics and hypertensive-diabetics were noticed. The following data was registered: DMFT=27.16 (± 6.15), with 22.94 (± 10.46) of missing teeth; T-Health=5.23 (± 6.52); FS-T=8.53 (± 10.12) and SiC=32 (± 0.00). 85.78% of the individuals were using prosthesis (58.72% Dentures) and 61.01% needed prostheses (58.26% in the jaw).The correlations between OHIP-14 (5.37 [± 4.95]) and oral health evidenced the increase in the number of teeth affecting psychological dimensions, besides the use and need of prostheses were associated to physical and social impacts (p < 0.05). We concluded that edentulism, use and need of prostheses affected quality of life in hypertensive and diabetic patients concerning psychological, physical and social aspects.
